At our practice, we prioritize delivering the highest quality care to our patients, which is why we have chosen not to be in-network with insurance providers. By doing so, Dr. Mikhli can ensure that patients receive the best possible treatment without being constrained by insurance approvals. Instead, we offer a diverse range of payment options and financing plans to accommodate individual needs.


Serenity, our Treatment Coordinator, will discuss the investment, payment options, insurance pre-determination, and reimbursements with each patient. We take care of all the necessary paperwork, including procedure codes and providing any required imaging, and promptly mail out the insurance forms.

Although we are not contracted with dental plans, our patients are satisfied with reimbursement. We estimate insurance reimbursements and savings for procedures like extractions and implants.


For patients without insurance, we have several ways to assist them in accessing affordable and manageable dental care.

We offer the following financial options:

  • SAVE 5%:  Payment in full at time of scheduling and save 5% off of your dental treatment.

  • 6 MONTHS INTEREST-DEFFERRED FINANCING:  Convenient dental financing through BlisPay, CareCredit or Lending Club. Financing offers include up to 6 months of deferred interest.

  • PAY AS YOU GO:  Payment in full for each procedure completed at time of service.

In addition to these options, we are proud to offer a lifetime dental implant warranty for all our implants, providing our patients with further peace of mind.

Financial Policy

Payment is expected as services are rendered. We accept Cash, Visa, MasterCard, American Express, Discover, CareCredit, LendingClub, BlisPay, and personal checks.

For those patients who are covered by insurance, we will be happy to bill on your behalf, whenever applicable.   Verification of benefits is not a guarantee of coverage. Medical necessity is up to the determination of your insurance provider. You, the patient, may be responsible for services, as Dr. Mikhli and Dr. Eisenberg are not contracted with any insurance company.


We will collect payment at the end of your visit, and submit an insurance claim on your behalf. We will not accept assignment of benefit from your insurance provider, therefore the reimbursement check will be mailed directly to you.


Any outstanding balance that is your responsibility will be expected to be paid in full within 30 days of notification.

Please understand that you will be expected to pay for all applicable fees the day of service. As the patient, you are responsible for the full cost of your visit. Dr. Mikhli and Dr. Eisenberg and their team can not guarantee any reimbursement from the insurance company.  As the patient, you need to assume responsibility of notifying Precision Implant Care of any changes in insurance coverage.

With the passage of the Health Insurance Portability and Accountability Act (HIPAA), the federal government has new rules about how physicians and medical practices use your personal medical information. Maintaining patient confidentiality and safeguarding the appropriate use and disclosure of this information have always been a part of our practice. the new law is largely a formal, regulated framework for privacy issues.


We are providing you with our formal Notice of Privacy Practices (click button below).  Please take the time to review it carefully.  The new law requires us to verify that we have provided this Notice to you today.  We would appreciate your signature acknowledging the receipt of the Precision Implant Care Notice of Privacy Practices.  The acknowledgement form is in your new patient paperwork packet.
